A patient with diarrhea
A 52 year old female patient is described. Six years earlier hypothyroidism due to Hashimoto’s thyroiditis was discovered. Moderately elevated aminotransferases were noted. When she presented with diarrhea, a polyp was detected at colonoscopy and removed with a snare.
Histological examination revealed invasive malignancy, so a surgical sigmoidectomy was performed. Liver biopsy revealed primary sclerosing cholangitis. Diarrhea recurred and signs of malabsorption appeared. Celiac sprue was diagnosed at duodenal biopsy. Glutenfree diet was started; the liver tests normalized, but skin lesions of dermatitis herpetiformis appeared.
Symptoms of celiac sprue in adults and associated conditions are discussed.
