Abstract :
De preoperatieve en intraoperatieve bevindingen van 300 patiënten die geopereerd werden voor primaire hyperparathyreoïdie worden samengevat. Daarop aansluitend volgen enkele algemene beschouwingen over de heelkundige methode.





Surgical treatment of primary hyper-parathyroidism
The paper gives a survey of 300 patients operated on for primary hyperparathyroidism. Age, gender and clinical presentation were reviewed in the preoperative files. The female/male ratio was 3/1. One third of the patients could be considered as asymptomatic as regards their hyperparathyroidism. During surgery a solitary adenoma was found in 90% of the cases, while multiglandular disease was seen in 10% (a double adenoma in 5.3% and hyperplasia of the four parathyroid glands in 4.3%). Patients exhibiting hyperplasia were, on the average, younger, the female/male ratio being 1/1.
The surgical procedure and especially the controversy between the traditional bilateral approach and the unilateral or minimally invasive techniques are shortly discussed.
