A patient with type 1 diabetes mellitus presenting with fever and abdominal pain
Diabetic patients are particularly at risk for emphysematous pyelonephritis, a rare life-threatening infection of the kidney characterized by the presence of gas within the renal collecting system, the renal parenchyma and the perinephric tissue. Urgent nephrectomy has traditionaly been considered as the treatment of choice.
We present a typical case of emphysematous pyelonephritis treated with antibiotics and temporary percutaneous drainage. Recent literature data endorse this more conservative approach.
