A 72-year-old male with colicky pain in the right lumbar flank
A 72-year-old male patient is admitted to the hospital because of acute colicky pain in the right flank, suggestive of a kidney stone. On physical examination, a centrally located abdominal mass is found. Ultrasound and computerized tomography confirm the presence of an aneurysm of the abdominal aorta, exerting extrinsic compression on the right ureter and thus causing a right sided hydronephrosis. The patient is referred to vascular surgery; the aneurysm is resected and replaced by a dacron aortic prosthesis.
This case illustrates the importance of a thorough physical examination when confronted with a patient with a rather benign symptomatology. Although initially suggestive of a kidney stone, this case turned out to be caused by a potentially life threatening condition.
