A 77-year-old patient with acute vomiting and abdominal pain
A 77-year-old male patient was hospitalized because of intestinal obstruction. Two weeks earlier, the diagnosis of a gallbladder stone had been made and a medical treatment initiated. On admission, abdominal X-ray showed signs of obstruction as well as the presence of a radio-opaque structure in the lower abdominal region. A CT-scan confirmed the diagnosis of an obstructive ileus caused by a migrating gallstone. Comparison of the abdominal X-ray with the one two weeks before allowed to confirm the migration pattern of the stone. Ileus caused by a migrating gallstone is also known as Bouveret's syndrome. It is a rather unfrequent complication of cholelithiasis. Diagnosis can often be suspected by the medical history and the review of abdominal radiographs, if available.
