A 55-year old woman with diffuse joint and muscular pain, high fever and numerous little red painful spots
The bowel-associated dermatosis-arthritis syndrome (BADAS) was originally described in patients who had undergone ileojejunal bypass surgery for morbid obesity. The syndrome is clinically characterized by fever, arthritis, myalgia and an inflammatory skin eruption, which consists of vesicles and pustules with the histopathological findings of a dense dermal neutrophilic infiltrate. Later BADAS was reported in other abdominal surgery and inflammatory bowel diseases as well.
It has been proposed that bacterial overgrowth in a blind loop or a diseased bowel segment could cause circulating immune complexes which may subsequently lodge in target tissues such as the skin and synovium initiating an inflammatory reaction.
