A 40-year-old man with low backache
We describe the case of a 40-year-old man known with cutaneous mastocytosis (a mixture of telangiectasia macularis eruptiva perstans and urticaria pigmentosa) who presented with a severe low back pain since 5 months. On standard X-rays and CT-scan of the spine, several vertebral collapses were noted. The assessment of bone mineral density was in the osteoporotic zone (T-score L1-L4: -3.2). Further investigations revealed an increased serum tryptase level. On bone marrow examination, numerous mast cells were seen. This confirmed the diagnosis of systemic mastocytosis, causing osteoporosis.
Osteoporosis in a man of 40 years old is never trivial and an underlying cause (hemochromatosis, multiple myeloma, hypogonadism, steroid excess, systemic mastocytosis,…) should be looked for.
