Chasing the dragon
A 40-year-old man was admitted to the hospital because of unresponsiveness after presumed inhalation of heroin. Brain MR imaging was highly suggestive of an acute leukoencephalopathy after heroin inhalation, however without involvement of the cerebellar white matter and the brainstem. The diffuse white matter involvement with sparing of the subcortical fibers is highly specific. The cerebellar grey matter changes may be related to post-hypoxic changes.
